Complex Needs Services
About the Complex Needs Service
Our Complex Needs Service provides support to people to improve and maintain health, well-being and social inclusion by; providing activities and opportunities within the service base and local community.
Examples of activities could include:
- swimming/hydrotherapy
- craft
- cycling
- physiotherapy and massage, armchair exercises
- music and dance
- accessing community venues for refreshments or lunch
- sensory room access
- basic cookery
There is an agreed weekly plan of activities within in each service which is reviewed at least annually and more often if required.

Trem-Y-Mor Complex Needs Service:
- Scarlett Ave, Port Talbot SA12 7PH
Eligibility criteria
The Community Connectors Service is available to people who have been assessed as having eligible social care needs.
Transport information
Transport to and from the service is not assumed function but, an assessed need. This will be considered during the assessment process. If you are assessed as requiring transport, this will be arranged for you by your social worker and the Complex Needs Service.
Paying for services
If you are eligible for the Complex Needs Service, there may be a charge. This will depend on your financial circumstances, what other services are being provided by the authority and the number of sessions that you require the service.
