The Community Resource Team (CRT) is a joint service provided by:
The CRT provides help to adults (over the age of 18) living in Neath Port Talbot who need support to stay independent, within their own homes.
Patients will receive:
- the right intervention
- at the right time
- from the right professional
You can contact the Community Resource Team through:
The Neath Port Talbot Adult and Children Single Point of Contact Team (SPOC).
The SPOC team coordinates both health and social care needs.
Our services
Sensory Support Team
A service to help people who are:
- visually impaired
- deaf or hard of hearing
- deaf-blind (dual sensory loss) a significant combination of hearing and sight loss
We support both children and adults with a sensory disability.
Community Reablement
A short-term support service to help you to learn or re-learn the skills needed for daily living.
Community Reablement can:
- help you to recover from an illness or a stay in hospital
- rebuild your confidence
- promote independence
- help you decide about your long term care needs
Community Occupational Therapy Service
We improve health and wellbeing through daily activities, using techniques, environment adaptation, and specialized equipment.
We can work with you to:
- improve your quality of life
- increase your independence and safety
- enable you to access other support services
- increase your choice and control
Assistive Technology
This service offers a range of categories to enable people to remain safe, secure and independent in and around their own home.
Assistive Technology can:
- offer simple to use equipment
- give peace of mind to the client, carers and family/friends
- reduce the level of support required to remain living at home
- provide 24 hour/365 day response from the monitoring centre
- promote independence
- reduce risks
Digital Solutions Suite
This innovative facility is designed to offer essential support and guidance to:
- people
- caregivers
- professionals
working within the field, who may benefit from using mainstream digital technology.
Appointments to visit the Digital Solutions Suite and view the assistive technology can be arranged by joining a group demonstration (max 6 places per session).
